reservation requirements
incoming KINDERGARTEN AND GRADE 1 STUDENTS
Reservation Requirements:
- For Kindergarten, the student must be 5 years old as of the last day of August
- Photocopy of school ID
- Photocopy of Recent Report Card (at least until 2nd quarter)
- 500 pesos reservation fee
- 100 pesos assessment test fee
INCOMING GRADE 7 STUDENTS
Reservation Requirements:
- Photocopy of school ID
- Photocopy of Recent Report Card (at least until 2nd quarter)
- 500 pesos reservation fee
ESC Requirements:
- One (1) Long Folder
- Original copy of PSA Birth Certificate
- Two (2) copies of 2x2 recent and identical pictures (white background)
- ESC Application Form and ESC Contract
- Latest Proof of Income of both parents: Latest Income Tax Return from BIR (ITR) or Certificate of Employment (COE) with compensation or Certificate of Indigency (persons who are retired with pension only or on call)
- Letter of Intent for ESC Subsidy (reason why in need of ESC subsidy)
INCOMING GRADE 11 STUDENTS
Reservation Requirements:
- Photocopy of school ID
- One (1) Long Folder
- Recent Report Card (at least until 2nd quarter) with school dry seal
- 500 pesos reservation fee
