Latest Proof of Income of both parents: Latest Income Tax Return from BIR (ITR) or Certificate of Employment (COE) with compensation or Certificate of Indigency (persons who are retired with pension only or on call)
Letter of Intent for ESC Subsidy (reason why in need of ESC subsidy)
